Support With:

  • Case management

  • Obtaining state IDs, apply for healthcare, essential items.

  • Peer support with recovery goals

We Also Provide Free:

Essential items and hygiene products, such as:

soap, shampoo, toothbrush/toothpaste, and laundry cards.  For our participants and patients, we have a Clothing and Gear Bank as well. 

More Connecting...

We can assist with:

  • government and community benefits (WA birth certificates, connecting with DSHS, SSI assistance, mobile phone set-up

  • housing/shelter (overnight shelter connection, finding transitional housing, permanent supportive housing, and financial help with rent & utility bills)

  • Seeking detox and/or inpatient treatment for substance use

Projects for Assistance in Transition from Homelessness (PATH)

A homeless outreach program which serves residents of Thurston and Mason Counties

First Contact

PATH's Staff is to be the first contact for adults experiencing homelessness (or at risk of homelessness), suffering from serious mental illness and may also have co-occurring substance use challenges.  

Continuity

Through a combination of steady outreach and consistent case management, PATH staff meet individuals “where they are” and assist in obtaining housing, rental assistance, financial resources, employment, educational goals and other essential needs.
